Change in Regular Collection Arrangements
Please use the Google Form https://forms.gle/17654SQipM8sYT5QA to notify the school of any change of regular collection arrangements for your child, for example, a play date. If arrangements apply to more than one child (3 siblings for example) please complete the form once for each child. Please complete the form NO LATER THAN 2PM where possible. Changes to collection arrangements after 2pm on the day should be communicated by telephone to the school office.
Consent to Walk Home Alone

If you child needs medication whilst in school, please click here to download the Medicine Administration Form. Once completed, please return the form to the school office.

If your child has an allergy or food intolerance, please complete this form and return it, together with a letter or report from a doctor or medical professional, to firstname.lastname@example.org.
